If you’ve been asking what DirectMax is, here is the short answer: it is a prescription erectile dysfunction (ED) tablet from Direct Meds that dissolves under the tongue. The seller says it combines three ingredients, sildenafil, tadalafil and apomorphine, and you get it after an online medical questionnaire and provider review.

DirectMax is a compounded tablet, not a generic Viagra or Cialis
DirectMax is a compounded product, and the FDA states that compounded drugs are not FDA-approved. A generic sildenafil or tadalafil tablet is a different thing, because it is an approved product with a full FDA label.
Direct Meds sells DirectMax as “DirectMax 3-in-1.” The seller’s pages describe it as a rapid-dissolve tablet marked “RX Only.” It is sold in Low, Medium and High strengths. Your prescription and pharmacy label set the exact amount of each ingredient.
You can see the seller’s own description on its Product Page.
What is in DirectMax?
Direct Meds lists three ingredients on the tablet’s packaging and in its benefit graphics. Here is how the seller describes the job of each one.
| Ingredient | What the seller says it does |
|---|---|
| Apomorphine | “Primes for arousal” |
| Sildenafil | “Maintains blood flow” |
| Tadalafil | “Extends effectiveness” |
The seller’s FAQ calls DirectMax a “physician-designed, multi-mechanism formula.” It says licensed physicians customize your dosage to your body and medical profile. The seller’s headline claims include “3x Stronger Than Others.” Those are the seller’s marketing statements.
Who makes DirectMax, and is Direct Meds legit?
Direct Meds sells it. Its terms describe the company as a technology platform that connects you with medical groups, providers and pharmacies, and say it does not practice medicine itself. Prescriptions are filled by licensed third-party compounding pharmacies, which can vary by location. Service is limited to states where it is offered, and you must be in the same state as your shipping address during the consultation.
On its reviews and contact pages, Direct Meds says it has served 300,000+ patients, holds LegitScript certification and has a 4.6 Trustpilot rating. Those are the company’s own figures. The company lists a physical address on its contact page. For more on complaints, ratings and billing, see our DirectMax reviews and legitimacy page.
Does DirectMax actually work?
The studies below are on sildenafil and tadalafil, and on sildenafil in dissolving forms. They are not studies of DirectMax. Here is what they found.
- Dissolving sildenafil film, 2018: A Frontiers in Pharmacology study compared three 50-mg sildenafil forms in 20 men with psychogenic ED. The under-the-tongue film gave faster early blood levels and higher 60-minute exposure than the other forms, and headache was less frequent in that small group.
- Dissolving sildenafil strips, 2021: A randomized trial in 120 men studied an orally disintegrating sildenafil strip.
- Sildenafil vs tadalafil, 2025: A 12-week randomized trial in 75 men compared on-demand sildenafil, on-demand tadalafil and daily tadalafil.
- Dose-response analysis, 2026: A network meta-analysis of double-blind, placebo-controlled trials found sildenafil 100 mg (odds ratio 9.06) and 50 mg (7.90) beat placebo for improved erectile function. Tadalafil 20 mg (7.13) and 10 mg (3.14) did too.
- Real-world comparison, 2025: A European Urology Focus analysis compared sildenafil, tadalafil and vardenafil using patient-reported outcomes from about 130,000 patients on a direct-to-consumer platform.
- Sildenafil plus testosterone, Harvard-reported: A trial in 140 men with low testosterone found that adding testosterone gel to Viagra made no difference. The lead author said “sildenafil works extremely well.” The same Harvard article notes that these drugs don’t work for about 30% of men who try one.
The seller’s FAQ says “no treatment works for everyone.” If you want to see how DirectMax lines up against other multi-ingredient offers, read DirectMax vs Rugiet Ready or DirectMax vs Quad by MEDVi.
How much does DirectMax cost?
Direct Meds lists per-dose prices by strength on its DirectMax FAQ (prices as of November 2, 2026; your checkout shows today’s price).
| Strength | Standard price per dose |
|---|---|
| Low | $8.78 |
| Medium | $9.28 |
| High | $10.78 |
Each strength comes in a standard 6-dose pack, and the FAQ says larger packs of up to 20 doses are available. The product page lists 6, 10 and 20-tablet quantities. The FAQ also mentions promotional pricing “as low as $7.45 per dose,” and the page banner offers 33% off.
The seller’s own pages present prices differently. The product page has shown “as low as $79.00” against a crossed-out $244.00, followed by “$119.00 every month thereafter.” The FAQ shows per-dose pricing instead. Before you pay, check that your checkout shows the strength, quantity, first payment and the amount of any later charge. Direct Meds says personalized pricing appears before checkout.
How does the online visit and prescription work?
You do need a prescription. DirectMax is labeled “RX Only,” and Direct Meds says treatment is prescribed and managed by a licensed clinician who reviews your health history. The steps are:
- Complete the secure online medical questionnaire.
- A licensed provider reviews it and, when appropriate, prescribes. The seller says there is no in-person visit.
- A licensed compounding pharmacy fills the order and ships it. The FAQ page says delivery takes 1 to 2 days. The seller’s reviews page says most patients receive a first shipment within 3 to 5 business days of finishing the medical evaluation.
Direct Meds says you have access to its nursing staff and doctors afterward through the patient portal. Support is at help@directmeds.com or (888) 696-7176, with hours listed as 9am to 9pm EST daily on one page and 24/7 on another.
What are the refund and cancellation terms?
Direct Meds’ terms (last updated April 16, 2026) set out the rules in plain terms:
- Subscriptions bill the total price every 30 days, with advance notification 5 to 7 days before each charge.
- You can cancel a subscription anytime, and the FAQ says membership plans can be canceled in your Patient Portal. There are no refunds for medication you have already received.
- For subscription products, the terms say auto-refill must be disabled at least 48 hours before the next processing date.
- After 24 hours, prescription orders generally cannot be canceled.
- Returns and exchanges are accepted only for incorrect medication, damaged products, products not meeting safety standards, unshipped products, or medication denied by a Direct Meds doctor.
In practice, if you pick a membership, note the date of the advance notice and switch off auto-refill at least 48 hours before the next processing date.
What are the side effects and who should avoid it?
Direct Meds lists these as the most commonly reported side effects, and says most are temporary and mild:
| Side effect | Listed by |
|---|---|
| Headache, flushing, nasal congestion | DirectMax FAQ |
| Mild indigestion, nausea | DirectMax FAQ |
| Dizziness, back or muscle discomfort | DirectMax FAQ |
| Temporary vision changes (blue tint or mild blurring) | DirectMax FAQ |
| Mild irritation under the tongue | DirectMax FAQ |
Safety warnings you need to know
Because DirectMax contains two PDE5 inhibitors (sildenafil and tadalafil), the warnings on their labels matter here. Direct Meds’ safety notice says not to use ED medications if you take nitrates (for chest pain) or guanylate cyclase stimulators such as riociguat. The combination can cause a dangerous drop in blood pressure. The same notice asks you to tell your provider about every medication and supplement, and about heart problems, low blood pressure, recent heart attack or stroke, kidney or liver disease, and eye conditions.
Seek emergency care for an erection lasting more than 4 hours (priapism), sudden vision loss, sudden hearing loss, chest pain or severe dizziness. MedlinePlus says the same about hearing: with sildenafil and tadalafil, a sudden loss of hearing, sometimes with ringing or dizziness, means calling your doctor immediately.
How does DirectMax compare with generic sildenafil or tadalafil?
| Question | DirectMax | Generic sildenafil or tadalafil |
|---|---|---|
| Ingredients | Sildenafil, tadalafil and apomorphine, per the seller | One active ingredient |
| Form | Rapid-dissolve tablet under the tongue | Standard tablets |
| Prescription | Required | Required |
| Price | $8.78 to $10.78 per dose standard, per the seller | Varies by pharmacy and plan; check your pharmacist or telehealth quote |
| Studies | Studies cover its ingredients and dissolving sildenafil forms | A 2026 network meta-analysis of placebo-controlled trials, a 12-week trial in 75 men and published FDA labels |
| Where to read the label | Your pharmacy label and prescriber | FDA labels at DailyMed |
If you’re wondering whether anything beats Cialis, the 2026 network meta-analysis reported sildenafil and tadalafil each against placebo, and the 75-man trial compared the two drugs with each other. Which one suits you depends on your health history and what you want from treatment, and that is what the provider review is for.
What are the symptoms of ED, and can it go away on its own?
ED means trouble getting or keeping an erection. Harvard Health notes that ED is increasingly viewed as a systemic medical problem. It is often due to atherosclerosis, the same artery process that usually comes before heart attacks and strokes. Harvard says that if trouble persists while other things in your life are stable, a conversation with a doctor is in order, and a testosterone blood test is a good next step.
That conversation is your one next step, whether or not you go on to order DirectMax. Bring a list of everything you take, including any chest-pain medicine.
My husband takes Viagra and doesn’t tell me. What should I do?
Keep the conversation about his health, not the pills. The nitrate warning above is a good opening, since it matters to anyone who takes ED medication. A calm, private conversation will usually go further than a confrontation.
Before you order
- Pick the strength and pack that match your prescription, and check the first payment and any monthly charge at checkout.
- Check which state your provider and pharmacy are licensed in, and confirm they cover your address.
- Read the pharmacy label when your order arrives, since it carries the amount of each ingredient.
